About Us
Conduit Health enables effortless access to essential medical equipment, supplies, and services for home-based patients with chronic conditions. As chronic care shifts from nursing facilities into the patientâs home, the ability to obtain necessary durable medical equipment (DME) becomes essential to keeping patients safe and supported at home.
Weâre tackling a complex challenge in a massive market: our target population receives over $500 billion annually in long-term support and services. The population eligible for Conduit Health is expected to grow by 50% to 24 million Americans over the next five years.
At Conduit Health, we combine user-friendly technology with telehealth to transform a highly frustrating process into a seamless experience for patients and their families, accelerating access to critical equipment and supplies and enhancing the quality and affordability of care.
Weâre rapidly scaling, with demand outpacing our capacity. Backed by top-tier seed investors (XYZ Ventures, Twelve Below, Eniac Ventures), we are a close-knit, NYC-based team with an office in Union Square, where we work in-person several days per week.
